In Beast of Reincarnation, Emma and Koo each have their own separate hunger meter. As long as the relevant meter contains food, that character gradually recovers HP while outside combat. An empty meter removes this passive regeneration entirely, leaving you far more dependent on Stimpacks, emergency medicine, and camps after taking damage. And you will be taking damage.
